The Behavioral Health & Wellness Support Center has recently assigned a specific staff person to each psychiatric hospital in the area. The role of this person is to provide assistance to DHS/Provider case managers - to effect rapid discharges for children in these hospitals. Staff in the Complex Case Management Unit will be available to attend the discharge planning meetings at the hospital; ensure smooth communication between the hospital, CBH, and DHS staff; help the hospital to understand DHS policies, practices, and procedures; and provide any other additional support as needed.

Social workers in the BHW Support Center-RTF/Complex Case Management (CCM) unit are also assigned to the various RTF agencies to act as liaisons with these agencies to trouble shoot problems or serve as a link between the agency and the DHS assigned SW for a particular child or youth. Please contact this group of staff if you receive a request or evaluation from the RTF for a step-down placement and they can assist you in the process which would include a referral to CRU requesting the step-down placement for the child. They can also assist if you receive a request for a lateral transfer to another RTF. Please call for any other additional support.
